It can be helpful to fill these out at your leisure and it's easier to take time to properly reference any materials you need to accurately enter the information.

New Patient Package

For follow-up visits, print pages 6-9 and have each parent and teacher complete his or her own assessment. You do not need to score the forms, just return them to LPC and we will handle the rest. We hope that this serves as a welcomed convenience in place of coming in to pick up hard copies.
